Three weeks. That is roughly how long most people sit on a mouth ulcer before mentioning it to anybody. It stings while eating, so they switch sides. Buy a gel from the chemist. Blame the heat, or the chili, or the new toothpaste.
And usually they are right. Most ulcers are nothing.
But a small number of them are not, and the trouble is that stage 1 oral cancer symptoms look almost exactly like the harmless stuff. No pain, often. No bleeding. Just something that will not go away.
What Stage 1 Even Means
At stage 1 the tumour is still small, around two centimetres or less, and it has not travelled to the lymph nodes. Which is good news, oddly. Caught here, treatment is usually simpler and survival figures are strong. Caught later… not so much.
So the whole game is noticing it early.
Stage 1 Oral Cancer Symptoms And The Early Signs Of Oral Cancer To Watch
Here is what turns up first. Some of these will sound too ordinary to matter. That is sort of the point.
- An ulcer or sore that has not healed in three weeks. This is the single biggest one of the early signs of oral cancer
- A white patch (leukoplakia) or a red patch (erythroplakia) inside the cheek, under the tongue, on the gums
- A lump or thickened bit of tissue you can feel with your tongue
- Rough or hardened skin inside the mouth that was smooth before
- Bleeding from the mouth without any obvious reason
Red patches worry doctors more than white ones, generally. Though both get looked at.
Early symptoms of oral cancer that get blamed on something else
The next lot are the sneaky ones, because every single one of them has a boring explanation available.
Numbness or a tingling feeling in the lip, tongue or chin. People assume it is a nerve thing from dental work. A sore throat on one side only, that just sits there for weeks – that gets put down to allergies or pollution, which in our city is a very easy thing to believe.
Then there is ear pain on one side with a completely normal-looking ear. Referred pain, it is called. The ear is fine; the tongue base is not.
Loose teeth with healthy gums also belong on this list, and so does a denture that suddenly stops fitting the way it used to. These are genuine early symptoms of oral cancer that get sorted out with a new denture instead of a biopsy.
Oral cancer warning signs that mean go now, not next month
Some oral cancer warning signs are a bit louder. Difficulty while swallowing, or you feel that something is stuck in the throat. Voice turning hoarse and staying that way past three weeks. Trouble opening the mouth fully. A swelling in the neck that does not hurt.
Weight coming off without you trying. That one gets missed all the time.
Why oral cancer early detection matters so much here
India carries a huge share of the world’s cases. Tobacco, gutkha, khaini, paan masala, betel nut, plus alcohol on top of it. Most of our patients are chewers, and a lot of them have been at it since their twenties.
The frustrating part is that the mouth is visible. You can see inside it in a mirror. Unlike most cancers, oral cancer early detection does not need a scan or a blood test to begin with, just somebody looking properly.
Five-year survival at stage 1 sits around 80 per cent or higher. By stage 4 it drops to somewhere near 30. Same disease. Different month of diagnosis.
Who should be checking their mouth regularly?
- Anyone who chews tobacco in any form, or has in the past
- Smokers, and heavy drinkers, and especially both together
- People over 40
- Anyone with a sharp tooth or badly fitting denture rubbing the same spot for years
- HPV-positive patients
If that is you, a quick self-check once a month under good light is worth doing. Cheeks, under the tongue, gums, roof of the mouth, and run your fingers along the neck.
Three weeks. That is the rule.
Honestly, the simplest way to handle all of this is a calendar. Anything in your mouth that has not sorted itself out in three weeks gets looked at by a doctor. Not a chemist, not Google. Most of the time you will be told it is nothing and you will go home relieved.
Occasionally you will not, and that is exactly the visit that makes the difference. Missing the signs of oral cancer rarely happens because the oral cancer symptoms were invisible; it happens because somebody decided to wait a bit longer. Same with mouth cancer symptoms in younger non-smokers, which get dismissed even faster.
Dr. Arvind Kumar Gupta, ENT and Head and Neck Consultant Surgeon, has spent over twenty years handling head and neck cases in the city, and sees early stage oral cancer symptoms regularly.
